Service Description
Depending on the chosen package, the client will receive several monthly wellness sessions. These sessions must be used within 30 days or will be forfeited. 24-hour notice is required for cancellations, and sessions will be rescheduled, provided the wellness professional has availability in their schedule.
Cancellation Policy
Because physical exercise can be strenuous and subject to the risk of serious injury, we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity. You agree that you do so entirely at your own risk by participating in physical exercise or training activities. You agree to voluntarily participate in these activities and assume all risks of injury, illness, or death. We are also not responsible for any loss of your peril property. CANCELLATION POLICY All cancellations must be received at least 24 hours before the scheduled time to avoid being charged for the session. If the client cancels in advance, every effort will be made to make up the session, provided the trainer has time available. No refund is available for any missed sessions due to hospitalization or long-term illness. Still, efforts will be made to reschedule, provided the trainer has time available within the weekly schedule. CONCLUSION I agree that no warranties or representations have been made regarding the results I will achieve from this program. I understand that results are individual and may vary. I acknowledge I have thoroughly read this waiver and release of liability and am signing this agreement freely and wilfully and not under fraud or duress. By signing this document, I am waiving any right, or may heirs and/or assigns may have to ring any and all legal actions or assert any and all claims against the trader, its respective representatives, executors, and /or assigns.
